At Community Care we understand that finding the right home care solution for a loved one can be
stressful and confusing. As your partner in care, we are here to help guide every step of the
way. Listed below are some steps that will help you get started.
Identify the need for care – click here to learn the warning signs that a loved one needs home health care.
Seek advice – Whether it is speaking with family, friends, peers, advocacy groups, state or
local agencies or Elder Law Attorneys, explore your financial options as well as options for
care. Do your research.
Discuss the situation with your family members – Make sure to include the key family
stakeholders in the process. Ask for their help and input in deciding what care is right for
your loved one. They should play a part in gathering information and speaking with various
agencies and facilities.
Consult with the Patient – Once you have gathered all the information, if possible, include
the patient in the conversation. Having them involved in the decision gives them a sense of
inclusion and usually leads to better long-term outcomes.
Determine what care is needed – Once you have discussed the situation with your doctor and
family you will need to decide what kind of care is required to assist with activities of
daily living (ADL). How much care, how often and what are the patient’s needs.
Review your payment options & budget – Medicaid, Medicare, Workmen’s Compensation
Insurance, Veterans Administration (VA) Benefits and Private Pay are options for funding
home care. You will need to determine which option is best for your specific situation.
Speak with your Doctor – Depending on insurance and payment options your Doctor may need to
prescribe home care services.
Choose the best option for you – Every patient, family and case are unique. For most
patients home care is the right solution. It allows patients to have the freedom and comfort
of living in familiar surroundings on their own schedules. In some cases, long term care
facilities may be right choice for certain patient’s needs.
Select the right agency – Make sure the provider you select takes your insurance. Make sure
they are licensed to provide the care your family member requires. The home care company
will schedule an intake call where they will gather all the pertinent case information and
assign an Aide that matches the care required.
Stay involved – You are the customer. Make sure you are happy with the Aide assigned to the
case. Touch base with the home care agency after start of care for updates and progress
reports.
