If more star were available she would get them!⭐⭐⭐⭐⭐ ⭐️ ⭐️ ⭐️I can’t say enough about Megan Shergill, the CDPAP expert at Community Home Care! She went above and beyond to assist a few of my clients who were struggling to navigate PPL, which has been a challenge for many. Thanks to her patience, expertise, and dedication, they were able to get the help they needed. Several of them reached out to let me know how amazing she was and how grateful they are for her support. Megan truly cares about the people she helps, and it shows in everything she does. I highly recommend Megan at Community Care Home Health Services and her team. Megan has been involved in many facets of the special needs community for many years and is extremely knowledgeable in the services that are available to support their needs. Megan and her team were very efficient in helping our family transition to the new CD Pap program. She is so compassionate and upbeat which made the process so much easier. I have been partnering with Community Care Home Health Services for several years in support of those that I work with who utilize OPWDD services. CDPAP is a great resource to supplement the OPWDD services that do not go far enough. Megan and her team are such an outstanding support for families! I can see where all the great reviews come from! I am in the process of transferring from another agency to community care. Megan and Christina have been AMAZING and so knowledgeable on services, when other agencies try to feed you false information. Community care are the best . I’ve worked with them over 7 years.I went to school for Nursing RN ..Very flexible. Hours The staff is very nice.pays well . They are very attentive to your needs. They are place you on Jobs that is convenient for you. They have bonuses and benefits also have plenty of cases they’re make sure you will not be out of work.. I recommend anyone who’s certified and wants to work in the Westchester area please stop by .

Yesterday was our first day with this agency. Unfortunately my mom was not feeling well with a stomach virus. The home health aid Delores and the the nurse Patty were both at our home to open our case with them. My mom was a mess. Delores who we had just met for 10 minutes before this episode began immediately sprung into action with care and concern. As well, Patty the nurse who was just there to do intake and assessment rolled up her sleeves and jumped right in to assist Delores in tending to my mom.The care, concern, diligence and teamwork that Delores and Patty displayed amazed me. If this is any indication of the standard of care that Community Care provides i'm sure my mom is in good hands. These two women were angels on earth for us yesterday, it really was truly amazing!

We offer a variety of home care services tailored to each patients’ specific needs with the goal of enhancing their quality of life.

CDPAP

The Consumer Directed Personal Assistance Program is a New York State Medicaid plan which allows consumers to hire family, friends or the Personal Assistant (PA) of their choice to provide services for help with activities of daily living (ADL). Recipients have the flexibility and freedom in choosing their own caregivers.

Companion / Homemaker

Companions provide non-medical services and socialization to individuals in their home or communities who need assistance with tasks and daily living activities.

Personal Care Aide

Personal Care Aides provide personal care activities of daily living, including bathing, grooming, dressing, feeding, shopping, light housekeeping, and medication reminders.

Home Health Aide

Home Health Aides provide personal care and support including all activities of daily living, bathing, grooming, dressing, feeding, shopping, and light housekeeping. They can assist with medications, take vital signs, perform range of motion exercises, and prepare and serve meals.

Skilled Nursing

Our licensed nurses provide clinical care to patients in the home or community setting. Nurses provide personalized medical care for surgery recovery, infusion, on-going chronic conditions, wound care, medication regimens, specialty services and assistance with doctor’s orders.

Waiver Services

These services provide support to children and adults with long-term needs and their families to enable them to remain at home and in the community. Waiver services are designed for people who, but for these services, require the Level of Care provided in a long-term care facility.
